 ● Click on Storefront

 ● Click on 'Theme' on the left, and Click on the 'Customize' button

 ● Select the page where you want to add the 'Order Lookup' form. Then drag the 'Order Lookup Pro' widget on the left, and drop to the widget to the area you would like to form to be added. 

 ● Done
